Active member forms

Form TitleDescription
Spousal and Beneficiary Designation FormKeep your spouse and/or beneficiaries up to date. Active and deferred members can use HOOPP Connect to update their information.
Waiver of Joint and Survivor PensionA retiring member and his or her spouse use this FSRA form to waive their right to a survivor pension. This link will take you to the FSRA website.
Waiver of Pre-Retirement Death BenefitIf you are a spouse of a member and want to waive your right to a pre-retirement death benefit. This link will take you to the FSRA website.

Call Member Services at 1-877-43HOOPP (46677) to obtain the following forms.

Declaration of Spousal Relationship for Surviving SpousesIf you are the surviving spouse of a member and want to apply for a survivor benefit.
Buyback Quote Request - Transfer of Past Service from Former Pension PlanRequest a quote for a buyback that would involve transferring funds from a previous registered pension plan to HOOPP.
Disability Benefit Appeal ApplicationMembers complete this form to appeal their application for a disability retirement or a disability benefit which was denied.
Application for Shortened Life ExpectancyWithdraw the value of your pension if your life expectancy is less than two years.

Application for Disability Benefits (Member)Apply for disability benefits from HOOPP.
Attending Medical Practitioner’s AssessmentYour medical practitioner completes this form to support your application for disability benefits from HOOPP.
Letter of Authorization Provide HOOPP with consent to disclose information to a third party.

Retired member forms

Form TitleDescription
Spousal and Beneficiary Designation FormKeep your spouse and/or beneficiaries up to date.
TD1 forms on Canada Revenue Agency’s websiteManage the amount of tax deducted from your pension. This link will take you to the CRA website.
TP-1015.3-V form on Revenue Quebec's websiteClaim federal and provincial tax credits if you live in Quebec. This link will take you to the RQ website.

Non-residential banking informationIf you reside outside of Canada and want your HOOPP pension to be deposited into a foreign bank account.
Post-retirement Spousal Benefits Application (New Spouse)If you have a new spouse after retirement and want to provide them with survivor benefits.
Re-enrolment Estimate Request FormIf you plan to return to work and would like to know your options.
