Active member forms

Form Title Description
Spousal and Beneficiary Designation Form Keep your spouse and/or beneficiaries up to date. Active and deferred members can use HOOPP Connect to update their information.
Waiver of Joint and Survivor Pension A retiring member and his or her spouse use this FSRA form to waive their right to a survivor pension. This link will take you to the FSRA website.
Waiver of Pre-Retirement Death Benefit If you are a spouse of a member and want to waive your right to a pre-retirement death benefit. This link will take you to the FSRA website.

Contact Member Services to obtain the following forms.
Declaration of Spousal Relationship for Surviving Spouses If you are the surviving spouse of a member and want to apply for a survivor benefit.
Buyback Quote Request - Transfer of Past Service from Former Pension Plan Request a quote for a buyback that would involve transferring funds from a previous registered pension plan to HOOPP.
Disability Benefit Appeal Application Members complete this form to appeal their application for a disability retirement or a disability benefit which was denied.
Application for Shortened Life Expectancy Withdraw the value of your pension if your life expectancy is less than two years.

Application for Disability Benefits (Member)Apply for disability benefits from HOOPP.
Attending Medical Practitioner’s AssessmentYour medical practitioner completes this form to support your application for disability benefits from HOOPP.
Letter of Authorization Provide HOOPP with consent to disclose information to a third party.

Retired member forms

Form Title Description
Spousal and Beneficiary Designation Form Keep your spouse and/or beneficiaries up to date.
TD1 forms on Canada Revenue Agency’s website Manage the amount of tax deducted from your pension. This link will take you to the CRA website.
TP-1015.3-V form on Revenue Quebec's website Claim federal and provincial tax credits if you live in Quebec. This link will take you to the RQ website.
Power of Attorney Confirmation Form Advise HOOPP if you have appointed someone as your Power of Attorney for Property to act on your behalf for matters related to your pension.

Contact Member Services to obtain the following forms.
Non-residential banking information If you reside outside of Canada and want your HOOPP pension to be deposited into a foreign bank account.
Post-retirement Spousal Benefits Application (New Spouse) If you have a new spouse after retirement and want to provide them with survivor benefits.
Re-enrolment Estimate Request Form If you plan to return to work and would like to know your options.
