Authorizing others to support you
Over the course of your life, there may come a time when you need someone's assistance to manage your personal affairs, or you may want to authorize HOOPP to share your private pension information with a third party.
HOOPP’s ability to assist that person or organization will depend on their role and the authority they’ve been granted in each of the following cases:
Authorized representative
You can authorize HOOPP to disclose your personal information, and to discuss any matters related to your HOOPP benefits, to a third party. To designate this representative, you can either send a written request that is signed and dated, or you can complete a Letter of Authorization (a copy of this form is available on HOOPP Connect or by contacting our Member Services team).
Power of Attorney (POA) for Property
A POA for Property is a legal document that gives someone you trust the right to make financial decisions for you. Depending on their range of responsibilities, your POA for property may be authorized to obtain information related to your HOOPP benefits, update your contact or banking information, or provide certain direction to HOOPP on your behalf. Before HOOPP can begin to take instructions from your POA, we require a copy of a valid POA document. We may also require additional documentation depending on your situation. If you have questions or need assistance, please contact our Member Services team.
Statutory guardian
A “guardian of property” is someone who is appointed by the Office of the Public Guardian and Trustee (OPGT) or by the courts, to manage your financial affairs. The guardian may be authorized to obtain information related to your HOOPP benefits, update your contact or banking information, sign and obtain forms on your behalf, etc. HOOPP requires a copy of the Certificate of Appointment of Statutory Guardianship or a corresponding court order.
Keep in mind that a POA is generally not permitted to:
- change HOOPP beneficiaries
- direct HOOPP benefits to a bank account that is not in the member’s name
This document provides a simplified overview of HOOPP's benefits based on the terms of the HOOPP Plan Text at the time of publication. From time to time, HOOPP may amend the HOOPP Plan Text. In cases where the information provided in this document differs from that contained in the HOOPP Plan Text, the HOOPP Plan Text will govern. More details, including the full HOOPP Plan Text and a complete description of the Plan and its benefits, can be found on hoopp.com.
