To take the next step in your journey with HOOPP, simply complete the form below. As a reminder, you must be an Ontario-based physician operating under a Medicine Professional Corporation (MPC) to join. If you are a salaried employee of a participating HOOPP employer, you may already be eligible to join HOOPP. Reach out to your employer for information on joining. Not currently eligible? Please keep us in mind if your situation changes.

If you haven’t already, please take a moment to review information about HOOPP and the pension offering to confirm your interest in joining.

Once you’ve submitted the form, a HOOPP representative will contact you to:

  • share additional information about the Plan,
  • review your individual circumstances, and
  • help you decide whether to join HOOPP.
This document provides a simplified overview of HOOPP's benefits based on the terms of the HOOPP Plan Text at the time of publication. From time to time, HOOPP may amend the HOOPP Plan Text. In cases where the information provided in this document differs from that contained in the HOOPP Plan Text, the HOOPP Plan Text will govern.
